Abstract

결론
저자들은 주변의 정상조직을 보존함과 동시에 선택적으로 종양을 파괴시킬 수 있는 광역
학 치료법을 동물실험을 통하여 만족할만한 결과를 얻은 후, 임상적 치료방법으로 시도하였
다. 혀 우측부위에 발생한 초기의 편평상피세포암종에 대해 2차에 걸친 광역학 치료의 반복
시행으로 술후 조직검사에서 국소적 완치를 얻었으며 현재 환자는 정기적인 follow-up을 하
고 있다. 광역학 치료는 초기 두경부 암의 치료에서 유망한 방법이나 아직까지 그 기본적
기전은 완전히 이해되고 있지 않으며, 새로운 광감작제 등에 대한 연구가 계속되어야 한다.

The standard methods of treatment in oral and maxillofacial cencer are surgery,
radiotherapy and chemotherapy. But all of them can induce significant morbidities,
surgery in this area can cause remarkable junctional and esthetic problems. The
applications of photodynamic therapy(PDT) has been studying and trying in these days
to minimize post-treatment complications. It can almost directly affect tumor cells except
surrounding normal tissues, so we are able to reduce post-operative complications. But
there can be a couple of complications after PDT, as like skin photosensitivity, so
therapists must attend to its use and completely understand their mechanism before
doing it. In our clinic, PDT was tried in an early tongue cancer patient using topically
applied delta-aminolaevulinic acid (δ-ALA). The 64-year-old man was gargled with 5%
δ-ALA 4 hours before irradiation. The previous punched biopsy was reported as well
differentiated squamous cell carcinoma(T1N0M0). Light of 635nm wave-length from
argon-pumper dye laser was used. Total 150J/cm² was applied. After 10
days, PDT was applied again in a small suspicious area, 10 days after then, small area
was excised and examined histologically. It was revealed as cancer free. Now, the
patient was periodically checked up. Conclusively, PDT can be thought to be a
acceptable alternative and promising method in treating of early oral and maxillofacial
cancer.
